Translated arrival declaration forms
Please note: the translated paper arrival declaration cards listed below are samples only to assist you in completing the New Zealand Traveller Declaration paper form (English version) or Passenger Arrival Card (English version) when you arrive in New Zealand.
All travellers to New Zealand must complete a New Zealand Traveller Declaration before reaching passport control in New Zealand. You can complete a digital declaration at www.TravellerDeclaration.govt.nz, or a paper arrival declaration which will either be available on your flight or on arrival in New Zealand.
If you have completed a digital arrival declaration, you do not need to complete a paper arrival declaration.
The translated arrival declaration cards listed below are samples only to assist you in completing the New Zealand Traveller Declaration paper form or Passenger Arrival Card when you arrive in New Zealand. Only the card provided to you on the plane or at the border can be used to enter New Zealand, and answers must be provided in English.
Please note: Passenger Arrival Cards have been replaced by the digital and paper New Zealand Traveller Declaration, however, Passenger Arrival Cards may be provided on your flight to New Zealand and are still a valid arrival declaration for entry into New Zealand.
These translations below are samples only and cannot be presented to Customs or MPI staff upon arrival.
